ATTENTION Patients with Medicaid Insurance

 

Medicaid Managed Care transformation has started. Standard Plans went live on July 1, 2021. You may change your primary care provider twice a year. You may do so by calling your health plan. You may also change your health plan one time before 9/30.  Click here for additional information.

 

 

If you or your child have cough or shortness of breath or think you have been exposed to COVID-19, call us before your appointment.

 

 

COVID-19 Scheduling Rules

  • Thursday extended hours cancelled until further notice.
  • Open first, second and third Saturday of the month from 9:00 am to 1:00 pm.
  • No walk-ins visits allowed. Please call the office to schedule your appointment.
  • Same day sick visits available. Please call the office to schedule your appointment.
